how is insulin resistance diagnosed?
Posted on Dec 09, 2008 under insulin resistance symptoms | 4 CommentsI just went to the dr. who ran a blood test on me because my main symptoms are depression, difficulty losing weight, and becoming tired after eating. She wasn’t clear on what tests they were going to do. She is going to check my thyroid and run a cbc blood panel. I think she said they would also check my blood sugar. Is that how they check for insulin resistance, by checking your blood sugar? I ate a couple hours before that (which she knew because she asked me). Will that mess up the test? I thought you had to fast for 12 hours before getting checked for that kind of stuff.
There is only a thoroughly messy and difficult test called a eulycaemic clamp which directly measured insulin resistance (once took part in a clinical trial and had it done).
An indirect test can be as follows: presenting with at least 2 of the following 3
- waist circumference >90cm in women or >100cm in men.
- raised triglycerides
- raised fasting insulin
A blood test 2 hours post eating will not give a true fasting value but if it comes back high, then it will be repeated as a fasting test

Blood glucose = blood sugar. You can use that to check for diabetes or insulin resistance (prediabetes). You do not need to fast for blood sugar. You can diagnose diabetes in someone with a blood sugar of 200 or above nonfasting or 126 or higher in someone who has fasted. Fasting is more important for cholesterol panels.

You’re right. Insulin resistance, or pre-diabetes, is a more subjective diagnosis, for if your FASTING blood sugar is elevated, you already have the type 2 disease. (and all metabolic tests should be done after a 12 hr fast) Your 2 hour postprandial glucose drawn at the office may be elevated (diagnosis diabetes). low (possible insulin resistance) or normal.
Prediabetics are most often: overweight, (+) family history of DM, high triglycerides, and glucose often low or low normal, especially 3-5 hours after eating, causing the post prandial lethargy.

The CBC panel checks also for the HbA1c which is the percentage of glucose molecules clinging to the red blood cells over the past 90 to 120 days. That is how long red blood cells live. The percentage is normal ly between 4 and 6. And this cannot be changed in a few hours or weeks. It takes months to change this number.
The Cholesterol or Lipids Panel one must fast for! She will want one of those next time.
